Our ACP process is delivered in partnership with Fei Yue Community Services, a professional social service provider with deep expertise in end-of-life care conversations. Together, we create a tranquil, unhurried space for you to explore and document your healthcare wishes.
The process is officially recognised by the Agency for Integrated Care (AIC) and the Ministry of Health (MOH), ensuring your plan is accurately recorded within Singapore's national system and carries the weight it deserves when it matters most.
Appointing Your Nominated Healthcare Spokesperson
Without an Advance Care Plan, families often face life-altering medical decisions during moments of acute distress — without any guidance on what their loved one would have wanted. By completing an ACP and identifying your Nominated Healthcare Spokesperson (NHS), you give the people closest to you a clear, documented understanding of what quality of life means to you.

Advance Care Planning (ACP) is the process of reflecting on and documenting your personal values, beliefs, and healthcare preferences — particularly around end-of-life care — so that these wishes can guide the medical decisions made on your behalf if you are ever unable to communicate them yourself. In Singapore, ACP is recognised by the Ministry of Health and the Agency for Integrated Care as an important part of holistic healthcare planning. It gives your family clarity and confidence during what would otherwise be an overwhelmingly difficult time.
A Nominated Healthcare Spokesperson (NHS) is the person you designate to communicate your healthcare preferences to medical professionals on your behalf, should you lose the capacity to do so yourself. This could be a spouse, adult child, sibling, or trusted friend — someone who knows you well, understands your values, and is willing to advocate for your wishes.
Choosing your NHS and documenting their role clearly as part of your ACP is one of the most important decisions the process involves.TLL's Advance Care Planning service is delivered in partnership with Fei Yue Community Services, a recognised social service provider experienced in end-of-life care conversations. The process is facilitated in a calm, private setting and involves guided discussions about your healthcare values and preferences, followed by formal documentation within Singapore's national ACP system.
