What is an Advance Medical Directive?
An Advance Medical Directive is a legal document through which you provide written consent, in advance, to decline extraordinary life-sustaining treatment should you become terminally ill and unconscious, or lack the mental capacity to make or communicate medical decisions. It is a deeply personal declaration — one that removes the burden of this decision from your family and your doctors, and places it where it belongs: with you, whilst you are well and able to reflect clearly on what quality of life means to you.

What Families Ask Us About the Advance Medical Directive in Singapore
An Advance Medical Directive (AMD) is a legal document in Singapore that allows you to declare, whilst you are mentally competent, that you do not wish to receive extraordinary life-sustaining treatment if you become terminally ill and are unable to communicate your wishes.
It is governed by the Advance Medical Directive Act under the Ministry of Health. Any Singapore Citizen, Permanent Resident, or person residing in Singapore who is 21 years of age or older and of sound mind may make an AMD. It is worth considering at any age — the need for it can arise unexpectedly.Although both documents relate to your future healthcare wishes, they serve different purposes. An AMD is a legally binding document specifically governing extraordinary life-sustaining treatment in the event of terminal illness and unconsciousness.
An Advance Care Plan (ACP) is broader — it documents your general values, preferences, and wishes around end-of-life care, and guides healthcare professionals and your family in a wider range of medical situations.
The two documents complement each other and are often completed together as part of comprehensive legacy planning. TLC offers both services through The Life Legacy.TLL's role is to guide and support you through the AMD process — helping you understand what the document means, what it covers, and ensuring you feel informed and confident before proceeding. The formal AMD must be signed in the presence of a doctor and two witnesses in accordance with Ministry of Health requirements, and is registered with the MOH's AMD Registry.
